As the use of minimal access surgery for treatment of gynaecologic pathology has advanced, training of novice and experienced gynaecologic surgeons in these techniques has become paramount. Trainees benefit from standardized curricula to ensure sound skills and knowledge. Comprehensive understanding of preoperative evaluation, intraoperative decision making, and postoperative care are key. In this interactive session, speakers will share experience with integrating a training program locally, nationally, and internationally including successes and struggles. Experience with the Essentials in Minimally Invasive Gynaecologic Surgery (EMIGS) comprehensive program for the standardized education and assessment of MIGS knowledge and skills will be highlighted. Description of the EMIGS Didactics, EMIGS Cognitive, and EMIGS Manual Skills will take place in addition to the research that has been completed to advance and strengthen this training program. At the conclusion of this session, attendees will be equipped to incorporate and enhance a successful training program in minimal access surgery.

Current evidences are showing the impact of environment, both toxic chemical exposures or the endocrine disrupting chemicals (EDCs) and climate change on reproductive health (pregnancy as preterm birth, infertility, endometriosis, cancers and more). As such, as clinicians and healthcare workers, we need to increase the awareness and educate both our patients and the doctors in terms of environmental impacts on health. Definitely, as we try to tackle safety issues, toxins seen in our environment should be identified and measured. Hazardous levels could be set and it is the first FIGO endeavour to monitor toxicity standards in prenatal vitamins. Hopefully, this toxicity testing will open the gates to the other more daily commodities we use to be given the FIGO seal of safety. In Brazil, caesarean rates (CS) reached 57% in 2020 and analgesia rates during labour are low, especially in the public sector, where they are less than 20%. In France, epidural analgesia is used by 80% of women and CS rates are stable around 20%. Our aim is to increase the provision of analgesia during labour and to observe the impact of this measure on CS rates and women’s satisfaction in 2 public hospitals in Brazil supervised by 2 French centers. The fear of labour pain has been associated with a greater demand for CS. We expect that increasing the offer of labour analgesia may contribute to decrease of CS rates.

This hands-on workshop will teach the attendee how to evaluate and treat a patient with chronic pelvic pain. Using simulation materials and models, participants learn how to perform a physical exam for female pelvic pain, trigger point injections for the pelvic floor and abdominal wall, transversus abdominis plane (TAP) block, and pudendal nerve block. Attendees rotate through stations with faculty to provide feedback, tips and tricks, and practical insight with a focus on physical examination skills and adjuncts to treatment of pelvic pain.

Voices from the Field, tells the story five OBY GYN physicians working to save the lives of delivering mothers in the most remote regions of the Amazon, Himalayas, India, Kenya- and surprisingly, the U.S. Post-partum hemorrhage is the number one killer of birthing mothers, and death is preventable, especially with the use of an 80 -cent device invented Dr. Thomas Burke, a Harvard professor and Director of the Harvard Human Rights and Global Health Dept. at Massachusetts General Hospital. The challenge remains however, that the international medical establishment has been reluctant to adopt the use of this device for lack of profit potential. Voices from the Field follows the struggles and successes of five courageous female doctors who employ this device and continue to save mothers in the most dire circumstances. Filming was completed all over the world during 2019 by an entirely female film crew from Idaho.

Effective triaging tools to timely assess facilities/staff/tools suitability for safe maternal/newborn Dyad care are lacking. Especially in low/middle resource countries (LMRC) and underserved rural areas staff burden of poor resource allocation and integration between primary care and hospitals increase childbirth morbidity and mortality.
We present: Our Prep-for Labor novel tool, for personnel use aim to fill this gap; rapidly triage patients by directed questionnaire to diagnose low and high-risk pregnancy leading to prompt action. Thereby in 75-80% cases normal labor can progress unimpeded, while in high-risk cases decision whether patient can stay on site or transfer is required is made.
Based on triage we assess 1) premature labor management, 2) labor induced or spontaneous 3) C-section and TOLAC 4) newborn care management. This simple, readily implementable triage tool combined with patient’s evaluation determine whether care appropriate at current facility or transfer required where advanced resources for the dyad can be provided.

Minimally invasive surgery (MIS) is an occupational hazard with up to 50% of laparoscopists experiencing physical strain that persists beyond completion of a procedure. This is due to repetitive movements in prolonged, awkward positions that require high cognitive loads without breaks. Ergonomic interventions have been shown to effectively reduce the risk of MIS-related injuries, resulting in decreased lost workdays, cost, and surgical waste. The objective of this session is to 1) review optimal intraoperative techniques such as operating room set up, posture, and instrument selection and 2) identify preventative measures including stretching, microbreaks, and ergonomic assessment of the workplace.

The last few decades have seen a substantial rise in the use of caesarean section globally, with rapid increases in low- and middle-income countries (LMICs). Informed consent is an important prerequisite for surgical procedures, including caesarean section, and is an important component of respectful care. There is existing evidence of sub-optimal consent procedures, but there is limited evidence on potential interventions to improve these procedures.
We will begin this breakfast session by setting the scene about what is known regarding informed consent practices in relation to caesarean section in LMICs using evidence from the literature and from the USAID- funded MOMENTUM “Safe Surgery in Family Planning and Obstetrics” in Democratic Republic of Congo (DRC), India, and Nigeria. We will then invite participants to share their experiences related to the drivers of optimal/sub-optimal informed consent practices in their settings and discuss recommendations on mechanisms that might be used to improve informed consent for caesarean section with a focus on the needs of pregnant women/people and their families.

Voices from the Field, tells the story five OBY GYN physicians working to save the lives of delivering mothers in the most remote regions of the Amazon, Himalayas, India, Kenya- and surprisingly, the U.S. Post-partum hemorrhage is the number one killer of birthing mothers, and death is preventable, especially with the use of an 80 -cent device invented Dr. Thomas Burke, a Harvard professor and Director of the Harvard Human Rights and Global Health Dept. at Massachusetts General Hospital. The challenge remains however, that the international medical establishment has been reluctant to adopt the use of this device for lack of profit potential. Voices from the Field follows the struggles and successes of five courageous female doctors who employ this device and continue to save mothers in the most dire circumstances. Filming was completed all over the world during 2019 by an entirely female film crew from Idaho.

Hysteroscopy has gained a very important role in modern gynecology. Multiple advances both in technology and techniques have occurred. The aim of this session is to provide an update on the current role of hysteroscopy in modern gynecology with a fresh look into the future. This Hands-On Simulation Lab will cover basic and advanced hysteroscopy skills including office-based hysteroscopy, flexible hysteroscopy, operative hysteroscopy completed via resectoscopy or hysteroscopic morcellation, endometrial ablation, and transcervical fibroid radiofrequency ablation. Working stations will be set up and attendees will rotate through the stations with supervision of faculty to provide feedback, insight, and the latest evidence-based medicine. Focus will be placed on practical skills that can be incorporated into patient care in the office and operating room.

Differential diagnosis of chronic pelvic pain is among the most challenging clinical assessments performed by gynaecologists. Due to myriad of different organic systems (reproductive, urinary, digestive, musculoskeletal etc.) contained in the pelvis, very different conditions may present with very similar symptoms. In this session, we will go through the most frequent gynaecologic and non-gynaecologic causes of chronic pelvic pain, with a practical approach focusing on the primary symptoms and physical examination findings. We will base the session on the concept that “if you need to call the presentation atypical to fit in your diagnosis, your diagnosis is likely to be done”.
